I hereby authorize the staff of Excel Speed & Fitness Training, LLC to act for me according to their judgment in any emergency requiring medical attention, and hereby waive and release Excel Speed & Fitness Training, LLC from any and all liability for any injuries incurred while training.

I have no knowledge of any physical impairment that would be affected by the above athlete's participation. I also understand that Excel Speed & Fitness Training, LLC retains the right to use any photographs and images taken of the athletes during training for promotional purposes.

Team training and 1-on-1 training is also offered. Please contact the main office for group and team proposals. For groups or organizations we can bring our program to you. We are mobile!

Individual Training 1-on-1 $40 person/day
Regular Training $25 person/day
Group Training 1-29 kids - $20 person/day 30+ kids - $10 person/day

Uniforms can be purchased for $25 per set.

Pay a month in advance for only $125 per month.
